AGING WAIVER
This program provides home and community based services to eligible persons age 60 or older that are clinically eligible for nursing facility care.
COMMUNITY HEALTHCHOICE
This is a Medicaid managed care for persons 21 years of age or older who are both eligible for Medicare and Medicaid. It is also for those who require a level of care consistent to that provided by a nursing home.
OBRA WAIVER
This program provides home and community based services to people age 18-60 with developmental physical disabilities to allow them to live in the community and remain as independent as possible.
INDEPENDENCE WAIVER
This program provides home and community based services for persons ages 18-60 with physical disabilities to allow them to live in the community and remain as independent as possible.
ATTENDANT CARE WAIVER/ACT 150 PROGRAM
This program enables persons 18-59 with physical disabilities to live in their own homes and communities. Participants have ultimate control of their services, including the right to recruit, hire, train, and pay their attendants.
OTHER FORMS OF PAYMENT ACCEPTED
In addition to the waivers listed, we accept private pay, veterans assistance, and private insurances.
